We have taken OT lead Live-in care and Physiotherapy lead rehab to turn it into (what we believe) is not only the best option, but the only option you need when it comes to intensive at-home recovery. This can be recovery from surgeries, stroke, delirium, fractured hips, the list goes on.

BUT not only do we provide this service, unlike a lot of NHS commissioned rehab, we have no exclusion criteria. Just because your loved one is on their dementia journey does not automatically mean they can’t regain the ability to walk after falling and fracturing their hip. So how has an OT lead CQC registered home care provider and a Physiotherapy lead rehab service come together to deliver this unique and exceptional service? Common goals! We both have decades of experience in care and care homes and have seen the same issues over and over again!

In care homes, visitation hours may be limited, and individuals may feel a sense of separation from their loved ones. In care homes, if you can’t get out of bed you are dependent on when staff are available to assist you with the most basic needs, wanting to go to the bathroom. Push a button and wait. Can’t hold it long enough, don’t worry you have pads on…. In care homes, personalised care means filling out a form for the CQC to read, but getting whatever agency staff is available to support that day. In care homes, individuals often have to adhere to a strict schedule and routine. In care homes, you are there to be looked after. Not to engage in therapy to regain lost skills. Care homes do their best and some are phenomenal at doing care, but they are not designed to be rehabilitative. Their goal is not to get you back walking or going to the local shop or even getting on a Cruise ship for a holiday.

You have a personalised Assessment and Treatment, your OT will conduct a comprehensive assessment of the individual’s needs, taking into account your physical, cognitive, and emotional abilities. We will evaluate your home environment and identify any barriers or hazards that may affect your independence and safety. If needed we can recommend home modification, equipment and adaptation.

One of the key advantages of having one of our OT’s prescribe care at home is their expertise in home modification and adaptation. We can recommend modifications to the home environment to promote accessibility, safety, and independence. This may include installing grab bars, ramps, or stairlifts, rearranging furniture for better mobility, or suggesting assistive devices to aid in daily activities. As OT’s we look at skill development and rehabilitation, Our  OT’s  are skilled in helping individuals develop or regain the skills necessary for independent living. We can provide guidance and training in activities of daily living (ADLs) such as dressing, bathing, cooking, and managing medications. Through our hands-on therapy and therapeutic exercises, your therapy assistant can help improve strength, coordination, balance, and cognitive function, facilitating your ability to carry out daily tasks.

Most importantly our OT’s will give your Therapy assistant individual training for working specifically with you. Having a Therapy assistant with you 24/7 for your rehab journey ensures proper techniques for assisting. From transfers, positioning, exercises, cooking, washing, dressing, community access and any other caregiving tasks. This training not only helps Therapy Assistants’ provide better care but also reduces the risk of injury or readmission to hospital, overall getting you to achieve your goals.   But it doesn’t stop there, at Sunday Care Therapy we have partnered up with the Caring Physio, a Physiotherapy lead home rehab provider, so let’s look at the advantages of having physiotherapy as part of your at home rehab program. Physiotherapists play a vital role in providing home care services to individuals who require rehabilitation, mobility assistance, and physical therapy.

From initial assessment and evaluation your Physiotherapists begin by conducting a thorough assessment of the individual’s physical condition, mobility, and functional abilities. They evaluate everything from your range of motion, strength, balance, and any specific areas of concern. This assessment helps your physiotherapist understand your individual’s needs and develop your personalised treatment plan. Your physiotherapist will look at mobility training, Physiotherapists assist individuals in regaining or improving their mobility. We will look at your pain management. Your Physiotherapist will help manage pain and discomfort through various techniques. They may use manual therapy, such as massage or joint mobilizations, to alleviate pain and improve joint mobility. Your Physiotherapists also educate your Therapy assistant on proper body mechanics, positioning, and postural exercises to reduce pain and prevent further injury.

As part of your rehab journey we understand the importance of ongoing monitoring and Adjustments: Your Physiotherapist will regularly monitor progress and make adjustments to the treatment plan as needed. They track improvements, assess any challenges, and modify interventions to ensure the best possible outcomes.
